6/4/2023 EnchantaVaughn Approved 1.Behavior: Ja’Carei appeared to be in a good mood being as though he was having a session with QMHP-C. QMHP-C informed Ja’Carei that this session was going to focus on his communication skills and making positive decision-making skills to reduce additional stressors and conflict within the home, school, and community setting. 2. Behavior: Ja’Carei was cooperative throughout the session as he did not need any redirection from QMHP-C. He presented behaviors such as “yes ma’am, thank you, and you are welcome. 1.Intervention: QMHP-C explored with Ja’Carei his triggers surrounding anger and processed with Ja’Carei how being aware of his triggers, gives him an opportunity to possibly better manage his anger symptoms. QMHP-C explored with Ja’Carei how his triggers can change and how verbalizing those triggers whether through journaling, talking to QMHP-C or to family members is a way to address those anger symptoms. 2. Intervention: QMHP-C redirected Ja’Carei to focus throughout the session and encouraged Ja’Carei to utilize the interventions from his previous sessions to manage his impulsivity. QMHP-C ended session by reviewing interventions with Ja’Carei and encouraging them to provide feedback on the session. 1.Response: Ja’Carei actively participated throughout the session and was receptive to QMHP-C’s interventions that were provided to him during the session. While exploring his emotional state, Ja’Carei stated that he was happy. Ja’Carei stated to QMHP-C that he will learn how to write things down in a notebook so that he can share with his counselor when she visits him. 2.Response: Mother reported that she really can see a little improvement within his behavior since services has started for he has not gotten in trouble at school yet. At the end of the session, Ja’Carei verbalized to QMHP-C that he is going to continue to do the right thing.
